Please note that the form on Clinical Portal is currently still called the "Anticipatory Care Plan Summary" as we are awaiting system updates to change the terminology. Therefore these guides will refer to Anticipatory Care Plans and ACPs rather than Future Care Plans. You can read about the change of terminology here.
